Pinal County Public Health seeks input on community health survey

FLORENCE – Pinal County Public Health would like to encourage residents to participate in a Community Health Needs Assessment in partnership with Sun Life Family Health Center.

  As part of the assessment process, Sun Life is conducting a community health survey to identify gaps and areas that need improvement in health care delivery systems in Pinal County.

  The community health survey takes 5-10 minutes to complete and is available online at the following links until October 31, 2016:

  Paper surveys will also be available at Sun Life offices throughout the county, and can be completed and turned in at that office’s front desk, or mailed directly to 865 N. Arizola Road, Casa Grande, 85122, ATTN: Renée Louzon-Benn. Sun Life has offices in Apache Junction, Casa Grande, Coolidge, Florence, Eloy, Maricopa, Oracle, and San Manuel. Paper surveys will be accepted until Oct. 31.
