Community health survey seeks public input

FLORENCE, Ariz. – Pinal County’s Public Health program is seeking input as part of a statewide health initiative with the Arizona Department of Health Services. The Community Health Assessment is an online survey that takes less than ten minutes to complete.

The survey asks questions about factors that affect perceptions of health and access to care. No personally identifying information is required.

The survey will be available online at SurveyMonkey.com/s/PinalCounty. SurveyMonkey is a free website that allows the user, in this case Pinal County Public Health, to build a survey, collect results and analyze the data later. Participating in the survey has no strings attached, will not result in email ‘spam’ and is a secure, safe site to offer your perspective anonymously.

Results from the study will be used to create a Pinal County and statewide Community Health Improvement Plan.

The survey will be available online until November 15, 2012.
